Program Description
If you reside in Ontario and have been prescribed any form of generic Pre-Exposure Prophylaxis (PrEP) medication, you may be eligible for support through the Freddie Financial Assistance Program for Emtricitabine-Tenofovir (the “Program”).
At PurposeMed, we are committed to ensuring that you get the support you need while receiving the generic PrEP medication you’ve been prescribed. That’s why we have created this Program – which is specifically designed to assist individuals that have been prescribed generic forms of PrEP medications.
How the Program Works
The Program provides financial assistance to eligible applicants in an effort to subsidize the cost of generic Pre-Exposure Prophylaxis (PrEP) medication. Eligibility and the amount of financial assistance that each such applicant is entitled to is based on a number of criteria, including, but not limited to, annual household income.
Programs Scope
In addition to the eligibility criteria noted above, this Program currently only applies to individuals that:

reside in Ontario, Canada; and
-
have been prescribed Generic Emtricitabine-Tenofovir.
Furthermore, this Program (including eligible medications) may change or end at PurposeMed’s sole discretion, at any time.
To apply for Freddie Financial Assistance coverage, please complete these three steps:

Complete and submit the following program application form to determine patient eligibility:

Mail the following documents to PurposeMed Inc. at 3200, 700 2 Street SW Calgary AB T2P 2W2

Official prescription receipt(s)

Payment receipt(s)

Reimbursement Process
PurposeMed will reimburse eligible Program applicants via mailed cheque within 8-12 weeks from the time PurposeMed receives the completed Program application form and the required documents.
